   A consultation is a free 15-20 minute meeting with the artist of your choosing. After completing and sending this form, Amulet Tattoos reception will be contacting you through email within 48 hours. We appreciate your patience as we are very busy.

*Filling out this form does NOT guarantee you an appointment. Once we agree to your request, you'll be asked to put down a non-refundable deposit either online or at the shop. The deposit is what holds your appointment. No deposit = No tattoo appointment.
